I think it's more a win-win situation
callers: for the same amount at least double (landline) or nearly 3 times (mobile) the call time (for German callers)
etv: no 50% (landline) to 66% (mobile) of the amount sacked in by the telco providers.
So with the same amount of money both sides win. Well, if that's bad, I don't know what's good.

I doubt that etv would improve any service for italian callers if nobody from italy calls.
To stop people calling will stop etv doing anything for italian callers.

Excuse me if I think that this explanation is a bit too far-fetched.
You're basically saying that, even without ETV Gold, basically ETV today would have no services apart from the very channel itself.
And I find this to be a tad hard to believe.
I'm much more inclined to stick to facts: there was a HQ stream before for everyone. There isn't now. There is for Gold members.

there was a lq and a hq stream for everyone and with gold a hd stream was introduced for the gold members
the HD stream moved on to a new project and the HQ moved to gold
without gold and the HD stream, the HQ stream would now be in the new project
So no change for free viewers that wouldn't have happened anyway

p.s. I see HD is back at gold, so it's HQ in the new project now, but I guess that'll change again for the start (note: it's a guess, I don't really know).
